A common model created and used by a lot of BusinessOptix users is a Forms Report Model.
Your form must be created to begin with and the form location must be specified.
Make sure to save the form before moving on to creating the report.
Open a new report model and under the report model name, tick 'Form Report'. 'Form location' will appear underneath with an edit link button next to it. From this button you can choose the form that was created and saved before.
Once the form model has been chosen, the name of the link will now be displayed next to the form location.
Now, save the report and view it through the HTML view.
